You can ask us to close your Grata Health account and delete the data associated with it at any time. This page applies to the Grata Health mobile app for iPhone and Android, published by Grata Health Inc. You do not need to have the app installed to make the request.
Deleting your account is permanent. You will lose access to your secure messages, appointments, care plan, and self-care history in the app, and this cannot be undone. If you have an upcoming appointment or an active prescription, please speak with your care team first.
Grata Health is a licensed healthcare provider. Federal and state law require us to retain your medical record — clinical notes, prescriptions, lab and test results, and clinical correspondence — even after you close your account. We keep that record for at least seven years after your last visit, and longer where the law of the state in which you were treated requires it. Billing and insurance records are kept on the same schedule.
Retained records are stored securely, are not visible in the app once your account is closed, and are never used to contact you or to market to you. We may also keep a limited amount of information where we are required to for legal, tax, security, or fraud-prevention purposes.
You have the right to request a copy of your medical record before you delete your account. See our Notice of HIPAA Privacy Practices for how to do that and for your other rights over your health information.
If you would rather remove specific information — a journal entry, a group post, or an uploaded photo — you can delete it in the app, or email support@grata-health.com and tell us what you would like removed. Your account stays open.
