Four things the portal does that a fax confirmation never could.
Every patient you have sent, with their current status — outreach in progress, first visit scheduled, treatment started, actively engaged — grouped into Needs action, In care, and Closed. Filters live in the web address, so the view you check every Monday is a bookmark, and it works on a locked-down hospital browser with nothing to install.

No. There is nothing to install and nothing for your IT team to approve. It is a website you sign into with a code sent to your work email, so most teams are up in an afternoon.
Yes, within the rules. Aggregate outcomes for your organization identify nobody. Patient-level detail appears only where that patient has specifically authorized sharing with your organization; everyone else appears as a row you can see but not open, with no reason given for why.
No. You see status, dates, and whether care happened — a first visit completed, treatment started, a visit missed. You never see notes, diagnoses, medications, or lab results, and there is no way to write to the record.
Whoever sends referrals and whoever asks how they went. Day to day that is usually care coordinators, discharge planners, and case managers; add a director or medical director on the shared dashboard link.
