By accessing or using the Grata Health website and telehealth platform ("Services"), you agree to be bound by these Terms and Conditions. If you do not agree to these terms, please do not use our Services.

Description of Services

Grata Health provides telehealth-based medication-assisted treatment (MAT) for opioid use disorder, including Suboxone prescriptions, care coordination, and ongoing patient support. Our Services are available to eligible adults (18+) in the states where we are licensed to operate.

Eligibility

To use our Services, you must be at least 18 years old and reside in a state where Grata Health is licensed to provide care. You must provide accurate and complete information during the intake process. Grata Health reserves the right to refuse service to anyone who does not meet eligibility requirements.

Medical Disclaimer

Grata Health provides medical services through licensed healthcare providers. Our platform facilitates access to care but does not replace emergency medical services. If you are experiencing a medical emergency, call 911 immediately. Treatment decisions are made by your licensed provider based on clinical judgment.

Privacy and Data

Your use of our Services is also governed by our Privacy Policy and HIPAA Notice of Privacy Practices. By using our Services, you acknowledge that you have read and understand these policies.

Intellectual Property

All content on the Grata Health website, including text, graphics, logos, and software, is the property of Grata Health and is protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works from any content without our prior written consent.

Limitation of Liability

To the fullest extent permitted by law, Grata Health shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our Services. Our total liability shall not exceed the amount you paid for Services in the twelve months preceding the claim.
