How It Works
Grata handles the rest. You stay informed.
Submit a referral at partner.grata-health.com in under two minutes — or fax: (757) 379-1595.
Our care team contacts your patient within an hour of receiving the referral — by phone, text, or email — to schedule a telehealth visit directly with them.
Most patients have their first appointment with a licensed Grata Health provider within 48 hours of referral — and almost always within the same week.
With patient consent, your office receives monthly progress notes covering medication, adherence, and any clinical flags — so you keep visibility long after the handoff.
Why Grata
Grata becomes your alumni's ongoing MAT prescriber — months and years after their last group — with progress updates back to your office.
12+ months
Long-term retention
Grata patients stay engaged in medication-assisted treatment well past the typical IOP completion arc — recovery as the long view, not a single episode.
Monthly
Alumni progress notes
Your office receives recurring updates on how IOP graduates are doing in maintenance — visibility long after they leave your program.
$0
Cost to your program
No fees, no contracts, no revenue-share. Grata Health bills the patient's insurance directly and handles benefit verification before the first visit.

No. Grata Health is built for the outpatient phase after your program — we pick up where your prescribing leaves off and continue the plan you've established. If your IOP doesn't have an in-house prescriber, we can also fill that role, but most of our IOP partners refer to us at graduation.
Most programs loop us in during discharge planning — typically the final two weeks of IOP. That gives us time to verify insurance, review the patient's record, and schedule the first telehealth visit before they finish the program.
With patient authorization, we send regular updates to your office covering medication changes, adherence, and any clinical concerns. Your team gets ongoing visibility into long-term outcomes for the patients you graduated.
If a Grata Health patient needs to step back up — to PHP, IOP, or detox — we coordinate the warm handoff with your admissions team and share recent clinical notes. Once they're ready to step down again, we resume MAT without re-intake friction.
Yes. Grata Health accepts Medicaid in every state we serve, along with most major commercial plans. We verify coverage before the first visit so there are no billing surprises for your graduating patients.
